BOXING COMBATANT<BR><BR>A Contestant in a boxing match, mixed martial arts, or kickboxer competition.<BR><BR>CERTIFICATION REQUREMENTS:<BR>Must be at least 18 years of age, submit to a thorough physical examination by a physician licensed to practice medicine, blood test results for HIV,HBV, and HCV, submit two photographs, provide a copy of Federal ID or completed form, and demonstrate experience as a professional or have fought 10 amateur bouts or received training under an established trainer/manager; and pay required fees pf $50 for application and $25 for licensing.)<BR><BR>EXAMINATION REQUIREMENTS:<BR>The combatant must pass an oral examination given by the ringside physician or Director (Commissioner of Commerce and Insurance or his designee.
