This version of the Clinical Experience Permit application is used for renewal of a Clinical Experience Permit, experience(s) in an additional school district, for a change of placement, or to convert a Short-Term Yearlong Residency Permit to a Long-Term Residency Permit. This permit must be issued prior to participation in a clinical experience within WV public school for the identified validity period. The permit includes clinical experiences for teachers, administrators and student support personnel.
